Abstract

Innovative funding models are increasingly being adopted across health care sectors to help improve patient outcomes and experience, enhance access to acute and community services across the care continuum, and improve the value of health care services and dollars. A bundle payment model involves the provision of a single payment for an episode of care in a given clinical condition or patient population, thus providing a single point of accountability for health care services that are delivered across multiple care settings.
