Abstract

Community and home health nurses have recognized case management as a core component of home care since the inception of home health agencies. As the interpretation of the reimbursement for coordination activities has broadened, agencies have been able to derive some payment for these services. This article describes the development of a nursing case management program that focuses on the nursing practice model. It discusses goals and criteria developed and the risk factors identified, along with the management and evaluation activities that are key to this case manage ment model.
