Abstract
Appropriate discussion and documentation of advance directives (AD) are important aspects of medical care and a responsibility of all members of the interdisciplinary team. AD are particularly important in the long term and sub-acute care settings as these populations tend to have poorer overall prognoses. While most nursing homes (NH) make AD a priority, there are often opportunities to improve both the rate and quality of AD completion which may vary among facilities.
