Abstract

1. 1. Sixteen cases of localized epilepsy from various causes were investigated by means of 302 dapth electrodas and 249 subdural plate electrodas. Investigations always includad full neurological, angiographic and iar studies, serial scalp records often over a number of years, and corticography. Activation and stimulation were carried out with combined scalp and dapth recordings. 2. 2. datails of eleven of these cases are reported. 3. 3. In fourteen cases there was histological examination of the cerebral areas which had carried indwelling dapth electrodas, and in one of these histological changes were observed. 4. 4. Sixty-nine spontaneous seizures were recordad, 44 from the scalp and 25 from the dapths. 5. 5. Ictal and inter-ictal abnormalities may fail to appear where there is severe localized brain damage, whereas they may be present in less damaged areas. This fact is well known in scalp recording; it is now shown to apply to daep structures as well as to the superficial cortex of the hemispheres. 6. 6. Certain hypotheses are given on the chronology of lesions and the relationship and response of different parts of the brain at different times. Observations are also mada on thalamic discharges.
