SESSION TITLE: Monday Electronic Posters 5 SESSION TYPE: Original Inv Poster Discussion PRESENTED ON: 10/21/2019 02:30 PM - 03:15 PM PURPOSE: Depression (DEP) is a common comorbidity in Chronic Obstructive Pulmonary Disease (COPD), and is potentially modifiable. Clinical burden of COPD-related hospitalizations (COPDRH) in patients with depression is not well understood. We used the National Inpatient Sample (NIS) database from 2016 to examine rates of COPDRH in depression and whether depression associates with increased risk of COPDRH. METHODS: Patients >40 years of age were divided into DEP vs non-DEP groups, as defined per Elixhauser comorbidity software. COPDRH were identified by ICD10 codes J40-J44 as principal diagnosis. Demographics, comorbidities, rates of COPDRH and associated length of stay (LOS) were compared between groups. Relationship between depression and COPDRH was analyzed by univariate and multivariate regression analysis. RESULTS: Out of 22,771,753 eligible weighted discharges in the year 2016, DEP was present in 14.8%. Table 1 compares risk factors, comorbidities and outcomes in DEP vs non-DEP. DEP had significantly higher rates of COPDRH (3.3% vs 2.7%, p <0.001) and increased LOS (4.4 vs 4.2 days, p<0.001). DEP was associated with an increased risk of COPDRH [OR (CI) 1.22 (1.20-1.24), p <0.001]. This relationship persisted even after adjustment for age, gender, race, current smoking, obesity, obstructive sleep apnea, pulmonary circulation disorder, insurance status and median household income [OR (CI) 1.11 (1.09-1.12), p <0.001]. CONCLUSIONS: DEP is associated with the increased clinical burden of COPD beyond traditional risk factors and comorbidities. Further prospective studies are needed to confirm our findings. It would be interesting to study the impact of effective depression management on COPDRH. CLINICAL IMPLICATIONS: Understanding the relationship between DEP and COPD would help clinicians in risk stratification of COPD patients.
