Abstract

Objective: To evaluate the prognostic factors and the treatment policy of primary breast cancer (BC) in elderly women. Methods: In a retrospective study BC patients (pts) reporting to our department were registered and followed in a data base. Tumor characteristics, surgical therapy and adjuvant treatment were recorded and correlated with the overall survival (OS). Results: Out of 1000 women 308 pts were older than 60 years and were analyzed for this study. The median age was 71.7 years (range: 60-88 years). After a mean follow-up of 86.73 months (±39.8) 191 women were still alive. Metastasis was diagnosed at 57 pts. The multivariate analyses of the prognostic parameters showed that axillary lymph node involvement (p=0.0004) and progesteron receptor (p=0.00,013) have a significant influence on a better OS. Irradiation after surgery has also an effectiveness for local control and for a longer OS (p = 0.001). Conclusion: Axillary lymph nodes and progesterone receptor were the most important prognostic parameter for OS. Adjuvant irradiation after surgery is an independent parameter to reduce relapse and mortality in elderly pts with BC.
