Abstract
Introduction: To compare the diagnostic performance and the safety of CT-guided percutaneous needle biopsy in haematological patients with biopsy performed in patients without known haematological malignancies. Methods: Retrospective study of all biopsy performed in haematological patients in our institution (Foch Hospital) between 2010 to 2014.Control group: biopsy performed the same +/- one day in non haematological patients. Demographic features of patients, platelet levels, size and depth of the lesion, type (nodule vs consolidation), calibre of the gun, number of samples were analysed. Diagnostic accuracy and complications (pneumothorax, alveolar bleeding) were noted. Results: Thirty-one patients and 58 controls. None of the analysed criteria was significantly different between the 2 groups, except rate of thrombopenic patients (6 vs 1 in control group). In haematological patients, a diagnostic was performed in 22 /31 cases (haematological malignancies, 9; lung cancer,6; infectious process ,6; organizing pneumonia,1) vs 53/58 in controls. Complications of the biopsy for haematological patients were pneumothorax, 9 (requiring chest tube,3), haemoptysis, 2; vs respectively 16,3;5 in control group. Conclusion: CT-guided lung biopsy is useful for the diagnosis of pulmonary manifestations in haematological patients when performed by experimented interventional radiologists .
