Abstract

The pancreas is an organ that plays an important role in the digestive system, including both endocrine and exocrine functions. We report a case of duodenectomy with whole pancreas due to cancer at Department of Gastrointestinal and Hepato-Pancreato-Biliary Surgery, Bach Mai Hospital, and reviewed the current literature on indications for surgery and treatment of postoperative metabolic disorders in these patients. Total pancreatectomy can still be performed in centers with adequate techniques, equipment and postoperative resuscitation monitoring. Long-term treatment after surgery is closely monitored to avoid consequences from endocrine and exocrine disorders due to lack of hormones and digestive enzymes of the pancreas. Treatment of diabetes should be closely monitored daily, insulin supplementation according to blood sugar and prevention of hypoglycemic episodes. Pancreatic enzyme supplementation daily at a dose of about 80000-15000 UI/24h, during or immediately after each main meal, adjust dose according to clinical symptoms of digestive disorders.
