Abstract

A follow-up and rehabilitation care team implemented concerted, cross-disciplinary support with a patient nearing the end of her life. Working closely with the patient and her family, caregivers provided personalised palliative care. This account by members of the team shows the importance of the technical and relational dimensions in this context.
