Abstract

BACKGROUND: The loss of the breast affects body image and decreased self-esteem, so whenever possible, in the treatment of breast cancer, it is essential that mastologist maintain this condition, it can do for the conservative treatment techniques oncoplastics, and among them, reconstructing breasts with silicone implants, whose main purpose for these patients is to keep the breasts as close to natural. DEVELOPMENT: For breast reconstruction with subcutaneous placement of the implants, the choice is made by biodesign mammary implants with polyurethane foam, as this layer will be incorporated as an intrinsic capsule of the dermis and the sheath of the pectoralis major, which will involve the entire implants, preventing its fall in future minimize capsular contracture, the rotation and displacement of the implants. The implants of polyurethane on the domestic market are presented on round and oval base; spherical, conical, anatomical and tear-drop profile; medial, superior, inferior pole, with five different types of implants, all of four different projection heights . CRITERIA: To choose the type of implants to be chosen, it is necessary to know if the patient: A) want to keep the same pattern (type and volume) of the breast. B) want to increase the volume and change the type C) want to increase the volume and do not change the type D) wants to reduce the volume and do not change the type E) wants to decrease the volume and change the type After this information to adopt the following criteria: 1 - according to the type of breast: a-type I and IV are used implants with a round base b-type II are used oval base (diameter greater horizontal than vertical) c-type III are used is oval base (horizontal diameter smaller than vertical) 2- according as projection: a) maintain the same projection b) increase the projection c) reduce the projection RESULTS: Based on these criteria, use will be implants: 1- type I and IV without changing the type MAXIMUM XH (250-620 ml) - MAXIMUM HI (225-560 ml) MAXIMUM MD (190-560 ml) - MAXIMUM LO (145-560 ml) 2- type II without changing the type: NUANCE XH (205-595 ml) - NUANCE HI (180-535 ml) - NUANCE MD (250-555 ml) - NUANCE LO (220-505 ml)) 3- type III without changing the type ENHANCE XH (220-470 ml) - ENHANCE HI (190-580 ml) - ENHANCE MD (180-620 ml) - ENHANCE LO (230-535 ml)) Any 4-type to increase the projection ADVANCE XH (150-655 ml) - ADVANCE HI (140-615 ml) - ADVANCE MD (125-545 ml) - ADVANCE LO (105-510 ml)) 5-Keep or acquire a conical shape, natural NATURAL XH (210-590 ml) - NATURAL HI (185-590 ml) - NATURAL MD (155-465 ml) - NATURAL LO (130-580 ml)) CONCLUSION: The use of implants in breast reconstruction is a procedure well established and increasing their use is enhanced by new mastologists little accustomed to their management, so it is important to know the criteria that best bring good results to obtain a good image body with consequent improvement in self-esteem of the patient with breast cancer was mastectomy.
