Abstract
A 57-year-old woman with history of flutter had a surface ECG suggestive of clockwise (CW) atrial flutter with positive F waves in leads II, III, and aVF and negative F wave in lead V1 (Figure 1). The complete absence of an R wave in lead I was noted. Chest x-ray film demonstrated dextrocardia. Echocardiography established complete situs inversus with otherwise normal intracardiac anatomy.
