Abstract

I read with interest the recently published article in the Journal comparing the hospital costs of open and endovascular abdominal aortic aneurysm repair by Dr Dryjski and colleagues. In their retrospective review they found that elective endovascular abdominal aortic aneurysm repair (EVAR) was significantly more expensive than open repair with respect to initial hospital costs. A decreased length of stay with EVAR only partially offset this increase, with the cost of the endograft itself accounting for 78% of total EVAR costs. Dr Dryjski’s study and its results are similar to those of our group published in 2002. We analyzed hospital costs as well as costs of secondary interventions and EVAR surveillance and found EVAR to be approximately $10,000 more expensive than open repair despite a substantial decrease in hospital stay. In our study the cost of the endograft accounted for 57% of total EVAR costs. I have two specific comments and questions regarding the present study:
