Abstract

espanolIntroduccion: entre los problemas relacionados con el medicamento (PRM)se encuentra la prescripcion potencialmente inapropiada (PPI)de medicamentos. Los errores en la medicacion son considerados eventos evitables y debido a la repercusion economica que tienen son considerados un problema de salud publica. Los criterios STOPP/START se convierten en una herramienta util para prevenir la prescripcion potencialmente inapropiada(PPI).Objetivo:estimar los costos asociados a las PPI segun los criterios STOPP/START en pacientes diabeticos internados en una institucion de tercer nivel de Cartagena-Colombia.Metodo: se trata de un estudio retrospectivo de corte transversalen el que se hizo una evaluacion economica parcial centrada en los costos ocasionadospor las PPI segun los criterios STOPP/START contrastando los escenarios en donde existieron PPI versus los escenarios en donde no existieron. Fueron incluidos los datos de todos los pacientes mayores de 65 anos diagnosticados con diabetes mellitus que estuvieron internados en una institucion de tercer nivel de Cartagena-Colombia, durante el primer semestre de 2017. Se estimo el exceso y la razon de costos entre los pacientes con PPI y sin PPI.Resultados: la prevalencia de PPI segun criterios STOPP fue del29 % mientras que segun criterios START fue del 69,6 %. Contrario a lo esperado, para todos los atributos de costos, el costo medio fue menor en aquellos pacientes que reportaron algun criterio STOPP. En los pacientes con PPI segun criterio START se presento omision en costos como era de esperarse salvo en hospitalizacion y UCI. Conclusiones: aunque la comorbilidad incrementa la polimedicacion de manera significativa y con ello un mayor costo, esta distorsiona el comportamiento esperado de los costos. EnglishBackground: among the related problems with the (DRM)medicine there is the potentially inappropriate prescription (PPI)of medicine. The errors in the medication of the patients are considered avoidable events that can be related to the professional practice and because of the economical repercussion that they have, they are considered a public health problem.Objective. to estimate the associated costs of the potentially inappropriate prescriptions according to STOPP/START criteria in hospitalized diabetic patients in a third level institution during the first semester of 2017.Methods: it is a retrospective cross-sectionalstudyin which a partial economic evaluation was made focused on the costs caused by the potentially inappropriate prescription according to STOPP/START criteria contrasting the scenarios where these inappropriate prescriptions are present versus the scenarios where were not inappropriate prescriptions. Thedata of all the patients older than 65 years old diagnosed with diabetes mellitus whowere hospitalized in in a third level institution during the first semester of 2017. It was estimated the overrun and the reason of costs between patients with PPI and without PPI.Results: the prevalence of PPI according to STOPP criteria was 29 %, whereas the prevalence of PPI according to START criteria was 69.6 %. Contrary to expectations, for all the attributes of costs, the medium cost was less in those patients that reported some STOPP criteria. On the patients with PPI according to START criteria there was an omission in costs as expected, except for hospitalization and ICU. Conclusions: even though the comorbidity increases the polymedication significantly and with that, a bigger cost, the presence of comorbidities distorts the expected behavior ofcosts
