The National Clinical Trial (NCT) identifier number for study 302 was omitted from both the abstract and the main body of the paper; the NCT identifier for this study is NCT01129011. Additionally, the NCT identifier number for study 301 is incorrectly stated in the abstract as NCT00527782. The correct identifier number for this study, as stated elsewhere in the article, is NCT00527787. The authors apologise for these errors. Correspondence to: Dr. J. L.
