Abstract

Background: Hypertension causes heart remodeling, characterized by increased left ventricular mass (hypertrophy). This changes will stimulate the activation of Matrix Metalloproteinase-9 (MMP-9), to prevent excessive remodeling process. Objective: Research on the correlation of left ventricular mass index (LVMI) and MMP-9 have been carried out in population with diverse LVMI, that leads to different conclusions. This research was carried out in hypertensive heart disease patients with concentric hypertrophy, in order to get a better picture of the correlation of LVMI and MMP-9. Method: This was a cross sectional study with consecutive sampling method. The subjects werehypertensive heart disease patients with concentric hypertrophy that came to Instalasi Pelayanan Jantung RSHS and met the inclusion and exclusion criteria. Result: This study was conducted in 35 subjects with median of LVMI measurement was 122.03 g/m2, MMP-9 plasma level was 84 pg/ml. Spearman rank test showed positive correlation of LVMI and MMP-9 plasma level (r = 0462, p = 0.005). MMP-9 plasma also obtained significantly different in the comparison group of mild and moderate hypertrophy (p = 0.019) as well as mild and severe hypertrophy group (p = 0.006). This study was not aimed to assess the activity of MMP-9. Myocardium is not the only organ that produces MMP-9, but the other source of MMP-9 have been excluded. Conclusion: There is a moderate positive correlation between LVMI and MMP-9 plasma level in hypertensive heart disease with concentric hypertrophy.
