Abstract

Objective To investigate the correlation between serum CD30, CD30L levels and pulmonary hypertension with chronic obstructive pulmonary disease (COPD). Methods Eighty patients with acute exacerbation of chronic obstructive pulmonary disease admitted to our respiratory department from January to December 2016 were selected as the study subjects.They were divided into COPD combined with pulmonary hypertension group and COPD group.Forty healthy people with normal pulmonary function were selected as the control group during the same period.The clinical data were collected and the levels of C-reactive protein (CRP), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), CD30 and CD30L in serum were detected.The correlation between pulmonary artery systolic blood pressure (PASP) and COPD was analyzed, and the risk factors of pulmonary hypertension in COPD were analyzed by logistic regression analysis. Results The forced expiratory volume in one second as a percentage of expected value (FEV1%pred), FEV1/FVC, PaCO2, PASP, CRP, IL-6, TNF-α, CD30 and CD30L in COPD group and COPD group with pulmonary hypertension were higher than those in control group (F=244.185, 138.299, 84.321, 139.267, 291.35, 349.21, 67.150, 79.654, 19.324, 207.066, all P<0.05). The PaCO2, PASP, CRP, IL-6, TNF-α, CD30 and CD30L in COPD group with pulmonary hypertension were higher than those in COPD group (t=4.264, 9.680, 21.249, 76.534, 7.691, 24.162, 5.620, all P<0.05). The serum levels of CRP, IL-6, TNF-α, CD30 and CD30L in COPD patients were positively correlated with PASP (r=0.58, 0.25, 0.34, 0.43, 0.41, all P<0.05). Logistic regression analysis showed that CRP, CD30 and CD30L were independent risk factors for COPD patients with pulmonary hypertension (χ2=19.425, 25.114, 6.452, all P<0.05). Conclusions CD30 and CD30L are independent risk factors for COPD patients with pulmonary hypertension.That early, dynamic and combined monitoring of CD30 and CD30L changes has important clinical significance for early diagnosis of pulmonary hypertension in COPD patients.
