In their study, the authors calculate creatinine clearance by a modification of the method of Cockcroft and Gault (2) that incorporates a standard ideal body weight of 70 kg: C(c) cr male = 140 - ageserum creatinine.The authors provide a convenient dosing chart for aminoglycosides in Figure 4 of their article.
