Solution: Discont inue h y d r o m o r p h o n e and m a g n e s i u m trisalicylate. Give i m m e d i a t e release morphine oral tablets 120 mg q4h ATC supplementing with 30 mg immediate release morphine qlh prn for two days. Then switch to controlled release morphine oral tablets* 240 mg q8h ATC supplementing with 60 mg of immediate release morphine qlh prn. If possible in two days change time intervals for controlled release to ql2h Aq-C.
