Abstract

The orthokeratinized odontogenic cyst is a rare developmental cyst of the jaws that presents with less aggressive behavior compared to odontogenic keratocysts. The treatment modalities range from conservative approaches to surgical removal. A 39-year-old female patient was referred to an oral diagnosis service for evaluation of swelling in the anterior portion of the mandible. Panoramic radiograph revealed a radiolucent area with well-delimited borders affecting the body of the jaw bilaterally and causing dental displacement. An incisional biopsy followed by marsupialization was performed. Histopathologic exam confirmed the orthokerinized odontogenic cyst diagnosis. The patient remained under observation and, since a satisfactory regression of the lesion was observed, no surgical intervention was performed. Seven years after marsupialization, there is no sign of recurrence and complete epithelialization of the cystic bed is observed.
