Abstract

This case focuses on how artery catheterization is affected by congestive heart failure by asking the question: Does pulmonary artery catheter (PAC) use improve clinical outcomes in patients hospitalized with severe symptomatic and recurrent heart failure? The ESCAPE trial was the first large, multicenter, randomized clinical trial designed to evaluate the use of PAC in heart failure patients. The study included patients with severe symptomatic heart failure with hospitalization for heart failure within the prior year, urgent visit to the emergency department, or treatment during the preceding month with more than 160 mg of furosemide daily (or equivalent). The trial failed to demonstrate a benefit of PACs. PACs are no longer recommended for guiding the management of patients with heart failure in the intensive care unit setting.
