Abstract
Development of papillary thyroid cancer (PTC) inside branchial cleft cyst (BCC) is very rare and may cause diagnostic and therapeutic challenges. We describe a 29-year-old female who presented with a gradually increasing painless cyst of the neck. The histopathological examination revealed PTC in both the lumen and lymphatic weaving. In addition, multifocal PTC of the thyroid was diagnosed following thyroidectomy. We conclude that despite normal thyroid on ultrasonography, a histopathological examination may reveal the presence of multifocal occult PTC in the thyroid. Therefore, following the diagnosis of PTC in BCC, completion total thyroidectomy should be the recommended procedure.
