Abstract
Penoscrotal transposition is an uncommon congenital malformation of the external genitalia, in which the scrotum is malpositioned superior and anterior to the penis. This anomaly must be viewed as part of a spectrum of abnormalities in scrotal development, ranging from bifid scrotum to the very rare condition of complete penoscrotal transposition (CPST). In this extreme degree of malformation, the scrotum is located cephalic to the penis. Few cases of CPST have been reported and even fewer images shown in modern urologic literature.
