Methods The current study included data collected on over 200 consecutive deceased clients in a community mental health setting over a period of two years. Clients residing in nursing home (SNF) facilities and those diagnosed with HIV and residing in related programs were excluded from the study reducing the N to 157. When Seriously Mentally Ill (SMI) clients were excluded the data on the remaining 123 deceased clients was analyzed. Average death age, type of diagnoses, coordination of care between mental health and primary health care providers, and physical health care goal on behavioral health care agency's treatment plan was also closely evaluated.
