Abstract

System failure has many different causes. This presentation identifies and illustrates these causes. An attempt is made to assess their relative importance. Major topics considered are the loss of fault coverage due to imperfect implementation information or because the failure modes are not accurately represented by the single-stuck fault assumption. New temporary failure characterizations derived from laboratory experiments, simulation and system data are also presented.
