Abstract

this issue of Medical Care, Braithwaite et al1 describe the use of a payoff time framework, which explicitly compares the benefits of following guideline-recommended care with costs in the context of screening guidelines for colorectal cancer among frail people with significant comorbid disease burden. In bringing a combined focus on these 3 concepts benefit-cost comparison, comorbidity, and frailty they bring a sharp focus on what has been a long-standing debate, rarely made explicit, and complicated by the number of dimensions that need to be considered.
