Abstract

Introduction: The aim of this study was to analyze the incidence and outcomes of community-acquired pneumonia (CAP) among patients with and without chronic obstructive pulmonary disease (COPD) in Spain in years 2016-17. Methods: We used national hospital discharge data to select all hospital admissions of patients aged 40 years or older which were hospitalized with a CAP diagnosis. We grouped admissions by COPD status. Results: CAP diagnosis was identified in 228,139 hospitalizations (COPD patients: 19.85%). Incidence of CAP were higher among COPD patients than among those without COPD (IRR 2.23; 95%CI 2.21-2.26). CAP was more frequently among men than women in both groups. Overall, patients with COPD were significantly older (77.14 ± 10.55 years) than those without COPD (75.91 ± 13.74 years) and had more coexisting comorbidities. Factors independently associated with IHM among COPD patients were: age, comorbidity, P. aerugiona isolation, pleural drainage and mechanical ventilation.
