• Advance care planning conversations empower shared decision-making among persons living with dementia. • Community-based advance care planning during the early stages of dementia may prevent avoidable care and promote quality of life. • Standard and skillful communication by providers with consideration of social determinants of health may support early dementia advance care planning. • Nurses are ideally situated to facilitate caregiver engagement and promote community-based early dementia goals of care conversations.
