Abstract

For each of us, there are a few cases that you just never forget. More years ago than I would like to remember, during my pediatric ICU rotation during residency, I took care of Jimmy, ⁎ Names changed. a 9-month-old, 22-pound infant. He and his mother had been in a frontal impact motor vehicle crash. The infant had just passed 20 lbs, and his pediatrician had (incorrectly) told his mother it was OK to change his car seat from rear-facing to forward-facing, and she had done so. During the crash, Jimmy’s shoulder straps slipped off and he was ejected into the back of the front passenger seat. He had a severe frontoparietotemporal cerebral contusion. His initial intracranial pressure was more than 70 mmHg. Jimmy did not survive. Riding in the driver’s seat, his mother had sprained her wrist but had no serious injuries.
