The presence of a mushroom shape alone should be considered an indication of probable melanoma and not a high-risk nevus, as suggested in Table 1 and Table 2. This sign is mainly present in large melanomas, and it is very rare in small melanomas
The mushroom shape, due to the Bruch’s membrane rupture, in our opinion, is less important than the internal reflectivity, because it is present in large melanomas but is very rare in small/ medium size melanomas, where the low reflectivity allows a diagnosis to be made
