Abstract

The aim of the paper is to describe the Long-term Care system of the Marche Region and to identify main challenges to ensure its sustainability in times of economic recession. Using administrative routine data of hospital discharges, the paper analyses possible reforms of the hospital networks in the Marche Region by identifying those potentially inappropriate hospital stays which could be shifted to alternative institutions, such as the so-called intermediate care institutions. Before concluding with some final remarks, the role and the support of the informal caregivers are discussed. It is presented the care model of Up-Tech, an intervention research project recently funded by the Ministry of Welfare aiming at supporting the family caregivers of Alzheimer Patient through a multi-component intervention, entailing the use of case manager social workers, preventive nurse visit and new Information and Communication Technologies.
