889 Combined Volume Reduction With the Loop and Leave Technique Permits Safe Endoscopic Management of High Risk Giant Gastric Polyps Suraj Gupta, Joshua Penfield, David Klibansky, Douglas A. Howell* Pancreaticobilary Center, Maine Medical Center, Portland, ME Endoscopic management may not require resection with a very low likelihood of malignancy and is challenged by size, thick stalks and inaccessible locations. We have experienced success in a series of huge hyperplastic polyps by reducing the size of the lesions with 1:10,000 epinephrine injection followed by placing 2 strangulated detachable nylon loops without resection. Three cases are presented with escalating difficulty and risk. All were successful without complication.
