Abstract

Purpose: To report a case of combined central retinal vein with transient central retinal artery occlusion in a young patient showing rapid recovery of both visual acuity and vascular occlusion.Method: Case report.Result: A 16-year-old man awoke with blurred vision in his right eye in the morning. Ophthalmological examinations revealed counting fingers vision and an afferent pupillary defect. Fundus revealed dilated and tortuous retinal veins, scattered intraretinal hemorrhages, segmental swelling of optic disc, retinal whitening in the right eye. Systemic evaluation was normal. His best-corrected vision improved from counting fingers at 50 cm to 20/30 in the next day under conservative treatment with intravenous fluid and vitamin B12 supplement, and to 20/20 in two weeks. Fluorescein angiogram showed no abnormal finding after 2-month follow-up.Conclusions: In this case of central retinal vein occlusion with transient central retinal artery occlusion, the patient showed sudden visual loss and rapid recovery of both visual acuity and vascular occlusion after spontaneous resolution of transient retinal hemodynamic block. This rare case should be borne in mind in daily practice when making a differential diagnosis.
