Abstract

A 78-year-old man presented with failure to thrive. Physical examination was unremarkable apart from cachexia. Laboratory work-up and imaging with pan–computed tomography scan with contrast were unrevealing. Endoscopic evaluation for occult malignancy uncovered scattered erythematous nodular lesions with dark purple pigmentation during colonoscopy (Figure A), focused in the cecum with a single lesion in the transverse colon. Histopathology showed colonic mucosa infiltrated by neoplastic cells with hyperchromatic, round to ovoid nuclei, and moderate amounts of eosinophilic cytoplasm, arranged in sheets (Figure B).
