Abstract
Key Points Closed‐loop communication (CLC) can reduce miscommunication and medical errors. This quality improvement study showed that CLC was poor at baseline, improved modestly with education and training, and peaked only with management accountability. Effective and safe cath lab teams have common goals, good communication, mutual respect, and good leadership.
