The significant technological advances madeduringthe past decade in the fields of endovascular surgery have revolutionized the approach to cerebrovascular disease. Neurointerventionalists no longer have to restrict their tools to devices designedforcoronaryvessels,butinsteadenjoyanarmamentarium of increasingly diverse and specialized instruments customized to accommodate the brain’s vasculature (10) .I n parallel to these technical advances, the short-term results of the International Subarachnoid Aneurysm Trial (6, 7) (ISAT) established equipoise between surgical clipping and endovascular coiling for intracranial aneurysmsinwhich thetreatingphysician thoughteithertreatment was a good option.
