Abstract
Radioimmunoassay (RIA)による抗二本鎖DNA (dsDNA)抗体の測定は,感度鋭敏な方法として古くから汎用されている.しかし従来のRIAの抗体測定範囲は狭いため,全身性エリテマトーデス(SLE)で多発する高値陽性血清の正確な抗体価の把握が困難であった.今回抗原dsDNA量を増加させ,測定範囲を拡大したRIAキットを開発し,その臨床的有用性を検討した.その結果,抗dsDNA抗体の測定範囲は旧RIAが0～100U/ml未満であったのに対して,新RIAは0～300IU/ml未満までの測定が可能であった.測定域の拡大により旧RIAで100U/ml以上の異常高値を示した血清39例中32例(82.1%)の最終抗体価の測定が新RIAで可能となった.また旧RIAで100U/ml未満の抗dsDNA抗体価は新RIAとの間にr=0.9346の良好な相関(p<0.01)を示した.測定値上限を拡大した新RIAは, SLE臨床像の解析や経過観察上にきわめて有用と考えられた.
