Advancing stroke diagnosis and management through nuclear medicine: a systematic review of clinical trials.
 • Aug 19, 2024
 • Frontiers in medicine
 • Hala F Azhari

Despite advancements in stroke care, challenges persist in timely triage and treatment initiation to prevent the burden of stroke-related disabilities. Although nuclear medicine has shown promise, no imaging technique has yet provided a sufficiently rapid, precise, and cost-effective approach to routine stroke management. This study aims to review the clinical application of nuclear medicine in stroke diagnosis and treatment. A systematic search of the Cochrane, EU Clinical Trials Register, ISRCTN, the International Stroke Trial, and the ClinicalTrials.gov database was conducted to find all registered trials reporting nuclear medicine's clinical applications in stroke up to June 07, 2024. Among the 220 screened trials, 51 (36 interventional; 15 observational) met the eligibility criteria. Participants were older than 18 years old, with only six studies including pediatric under 17 years old, with a total of 11,262 stroke (9,232 ischemic; 2,030 haemorrhagic) participants. The bias risk varied across trials but remained mostly low to moderate. The review highlighted nuclear medicine's significant contributions to stroke diagnosis and management, notably through mobile stroke units, pre-hospital acute stroke magnetic resonance image (MRI) based biomarkers, and MRI-based stroke mechanisms for 4D flow nuclear imaging. These advancements have generally reduced treatment delays and enhance clinical outcomes post-stroke. Specifically, radiopharmaceutical radiotracers can effectively discriminate between strokes and mimics, particularly in high-risk patients. Integrating novel positron emission tomography (PET) radiotracer 18F glycoprotein 1 and radionuclide angiography may improve sensitivity and specificity in thrombi detection for decisions regarding stenting or carotid endarterectomy, and the single-photon emission computed tomography and PET integration with ferumoxytol radiotracer-enhanced MRI enables functional imaging for evaluating cerebral perfusion, metabolic activity, and neuroinflammatory markers post-stroke. Overall, the integration of nuclear medicine into multimodal imaging equipment like computed-tomography PET and MRI-PET offers a more comprehensive picture of the brain. Nevertheless, further research is needed on novel stroke imaging techniques and standardization across stroke centers for optimal performance. https://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42024541680, identifier PROSPERO CRD(42024541680).

This study aims to explore the research landscape, hot topics, and future trends of non-pharmacological therapies for post-stroke spastic paralysis globally from 2000 to 2024 through a bibliometric analysis. We conducted a search in the Web of Science Core Collection database to analyze literature related to non-pharmacological therapies for post-stroke spastic paralysis published between 2000 and 2024. Tools including CiteSpace, VOSviewer, Bibliometrix, Scimago, and R language were used to identify and analyze countries, institutions, journals, references, keywords, as well as the most commonly used therapies and acupuncture points. The results were presented in the form of knowledge maps. The bibliometric analysis identified a total of 297 publications. Over the study period, the number of publications showed an overall upward trend. China had the highest number of publications. The journal *Archives of Physical Medicine and Rehabilitation* published the most articles. The most frequently occurring keywords were "stroke", "reliability", and "muscle spasticity." The most commonly used therapy was "acupuncture.". From 2000 to 2024, non-pharmacological therapies have shown positive effects in improving post-stroke spastic paralysis; however, more rigorously designed large-scale, high-quality randomized controlled trials are needed to confirm their long-term efficacy and mechanisms. Moving forward, international and domestic research institutions should strengthen collaboration to produce more impactful research and further explore individualized, precision rehabilitation treatment plans.

Astronomy classroom activities are usually attractive to elementary school pupils. As exploring astronomical phenomena, the pupils may have their doubts. Yet, not only students but teachers have difficulties in explaining basic astronomical phenomena(Baxter, 1989; Dai & Capie, 1990) This research aimed to develop a two-tier diagnostic test about and the units to reveal the misconceptions that elementary school teachers have in the convergence of the Earth and the universe viewpoints. The main objectives are: (1) to investigate the misconceived types and distribution of elementary school teachers in terms of and the moon; (2) to explore the differences under various conditions including academic background, science teaching experience, seniority of teaching science, having taught the or the lessons with the aid of the two-tier diagnostic instrument. In this study, the two-tier diagnostic instrument, invented by Treagust (1986), is developed into a three stages 14 steps. For the development of two-tier diagnostic tool, the misconceptions of starts and the from elementary school teacher via designed semi-open questionnaires is collected as a basic concept. The formal test took elementary school teachers in north part ofTTaiwan as participants and obtained 173 valid questionnaires. The statistical methods, such as descriptive statistics, T-tests, are used to analyze those questionnaires. That leads to the following conclusions: (1)Teacher have many misconceptions about the concepts of astrological observation, moving star, spectacle, revolution and rotation of the Moon and phase, date and location. (2)The most misconception type is non-conception type with wrong answer in two-tier questions, followed by the type first tier is correct and the second tier is incorrect, the least type is that the second tier is correct and the first tier is incorrect (3)The two-tier diagnostic test scores have no much difference from different academic backgrounds, but experience in teaching science influence test scores. (4)There is a significant difference in scores from teachers with varying experience in teaching science. The more experience the better score. (5)Science teachers who have taught stars unit or not does not make significant difference in the stars part scores of two-tier diagnostic test. (6)Science teachers who have taught moon unit or not does not make significant difference in the moon part scores of two-tier diagnostic test. (7)There is no much difference between science teachers who have taught moon or stars units or not. After in-depth analysis found that the experience in teaching science is proportional to the score. Science teachers who have over six years experience have higher score and less misconception. Suggested that schools should arrange regular science teachers. (8)The novice science teachers with mathematical background have more proper astronomical conception.

This study aimed to identify the mental health quality of life among patients with multiple sclerosis in Jordan. Thus, a descriptive quantitative design was used on a total of (N=100) Multiple Sclerosis patients that were randomly selected by using convenience sampling from the Health Insurance Center in the capital Amman, Jordan. Outcome measurement tools were the demographic data form and the Multiple Sclerosis Quality of Life-54 (MSQOL-54) Scale. The demographic data form consisted of questions about: age in years, gender, stage of multiple scleroses, and physical activities. The Multiple Sclerosis Quality of Life-54 (MSQOL-54) consisted of two domains the physical health composite and the mental health composite. In this study the mental health composite were used by the participants. The results revealed that the QOL- Mental Health Composite among patients with multiple sclerosis was 33.9 + 33.6. Moreover, there was no significant difference in score for male and females p=.874. In addition, there was no significant difference in QOL mental health scores for the age groups p=.165. Finally, there was a significant difference in scores for participants and non-participants in physical activity p=.000. Accordingly, this research concluded that Multiple sclerosis patients’ have a low quality of life in terms of mental health. In addition, practicing physical activities have a positive effect on the quality of mental health among multiple sclerosis patients.

Objective To evaluate the efficiency of sublingual immunotherapy with Dermatophagoides Farinae Drops in children with single and multiple respiratory allergic diseases. Methods Seventy-one children with allergic respiratory diseases who had been treated with Dermatophagoides Farinae Drops for one year or more were divided into a single allergic group (12 cases) and multiple allergic group (59 cases). The rhinitis score, daytime and night symptom score of asthma, VAS score, drug score, pulmonary function, and FeNO level before and after treatment were evaluated and compared between the two groups. Results The rhinitis score, night symptom score, VAS score, and drug score in the single allergic group after treatment were significantly lower than those before treatment (p < 0.05), but there was no significant difference in the daytime symptom score before and after treatment (p > 0.05). The rhinitis score, VAS score, and drug score in the multiple allergic group after treatment were significantly lower than those before treatment (p < 0.05), but there was no significant difference in the scores of daytime symptoms and nighttime symptoms before and after treatment (p > 0.05). In both the single allergic group and multiple allergic group, the pulmonary function indexes of the patients were significantly improved after treatment, and the FeNO after treatment was significantly lower than that before treatment (p < 0.05). There was no significant difference in scores, pulmonary function, and FeNO between the two groups (p > 0.05). Conclusion Sublingual specific immunotherapy is effective in treating multiple and single allergic respiratory diseases in children.

Transfusion of blood products is a common practice in anesthesiology. Inadequate transfusion medicine knowledge may lead to inappropriate transfusion practices and patient risk. Using a validated assessment tool modified for anesthesiology, we conducted a survey of anesthesiology residents in the United States to assess transfusion medicine knowledge. A validated transfusion medicine examination and accompanying survey were forwarded by program directors to residents for anonymous completion on May 5 and closed on June 30, 2021. The outcome of interest was the mean examination score. Secondary areas of interest were performance by year of training and previous educational experience in transfusion reported by the trainees. Rasch analysis was performed on the examination quality and individual question performance. Kruskal-Wallis H tests were used to identify differences between mean scores. Post hoc comparisons were used to assess specific pairwise differences between mean test scores by survey variable. Four hundred twenty-three anesthesiology residents in 37 programs completed the examination. The mean score was 45.5% ± 12.6%. There was a significant difference in mean cumulative examination scores between different resident training levels ( P < 0.001). There was a significant difference in scores between clinical anesthesia (CA)-1 and CA-2 residents ( P = 0.011) and CA-1 and CA-3 residents ( P = 0.012). No significant difference in examination scores was observed between CA-2 and CA-3 residents ( P = 0.95). All these subgroups scored below 50% on the examination. Significant differences between the residency training programs and cumulative scores were identified ( P < 0.001). This examination highlights gaps in transfusion medicine knowledge within US anesthesiology residents. Targeted education may improve knowledge in this area and patient care.

The objective of our work is to evaluate recovery induced by gabapentin alone and in association with intradermal infiltration of lidocaine in patients affected by moderate/severe tinnitus. Seventy-two patients suffering from moderate/severe unilateral non-pulsatile subjective tinnitus were enrolled. Severity and behaviour of tinnitus were assessed by Tinnitus handicap Index (THI) on the 8th, 22nd and 36th days from onset of therapy, and on the 3rd and 6th month after the end of therapy in patients treated with oral gabapentin (Group I), oral gabapentin and intradermal injection of lidocaine (Group II), and placebo (Group III) Significant differences in THI scores from the 8th day of therapy to the 22nd (p < 0.0001) and from the 22nd day to the 36th (p = 0.0002 and p = 0.0004, respectively) were found in Group I and Group II. In Group II, another relevant decrease of THI scores from the 36th day of therapy to 3 months from the end of treatment (p = 0.0004) was found. A significant difference in THI scores between Group I and Group II was found after 8 days of treatment (p = 0.05) with a more relevant decrease registered in Group II; significant differences were also found in THI scores between Group I and Group III after 8 days of treatment (p = 0.01), with a more relevant decrease registered in Group III; significant differences in THI scores between Group II and Group III were found after 36 days of treatment (p = 0.009), 3 and 6 months after the end of therapy (p = 0.005 and p = 0.007, respectively), with a more relevant decrease registered in Group II. In conclusion, the use of gabapentin associated to lidocaine seems to be superior to placebo and gabapentin in relieving tinnitus.
