Abstract
Objective To study the value of the fiber bronchoscope in treatment of severe pneumonia among the old. Methods 128 patients with severe pneumonia admitted in RICU were randomly divided into treatment group and control group,64 patients in each group. The patients were treated with oxygen absorption,anti-inflammatory and expectorant.etc, fiber bronchoscope was used for the treatment group to sputum aspirate and irrigate, while the sputum aspiration tube was used for the control group instead. The variation of SaO2,PaO2,and oxygen index after aspiration of sputum was observed, and the positive rate of sputum culture, time of the usage of antibiotics, and the average days of hospitalization between the two groups were compared. Results Compared with the control group, the arterial blood gas and oxygen index of the treatment group had improved (P <0.05).The total effective rate and positive rate of sputum in treatment group were 92.2% and 82.8%,which were higher than those in control group (73.4% and 34.4%,P <0.05).The time of the usage of antibiotics and the average days of hospitalization of treatment group were obviously shorter than those of control group (P < 0.05).Conclusions There is an exact clinical effect of using fiber bronchoscope to aspirate sputum and irrigate for treatment of severe pneumonia among the old.Because of a high positive rate of sputum culture, usage of fiber bronchoscope can shorten the time of the usage of antibiotics and hospitalization,it is worthy to application.
