The Clinical Knowledge Summaries guidelines (2007) on acute otitis media states, in its clinical scenario on persistent acute otitis media or treatment failure: ‘Consider sending a swab for culture and microbial sensitivity testing if there is perforation of the tympanic membrane that allows for a sample to be taken. This may be especially useful if first-line antibiotic treatment has failed.’ Regarding treatment of discharging otitis media, the GP Notebook (2007) states: ‘Swab should be sent for culture, and antibiotics commenced. Antibiotic therapy may be modified in light of the results of the bacteriology. There may be a requirement for regular aural toilet.’
