Abstract

Objective To investigate the clinical significance of urinary trypsinogen-2 assay test strip in the early diagnosis of acute panereatitis (AP).Methods Urinary trypsinogen-2 test strip was used in 500 patients with acute abdomen,and the results were compared with urinary trypsinogen-2 assay ELASA,urine amylase,serum amylase.Result According to the diagnostic criteria of Chinese Society of Gastroenterology of Chinese Medical Association,34 patients with AP were confirmed,urinary trypsinogen-2 test strip detected 32 patients and the sensitivity was 94%;urinary trypsinogen-2 test strip detected 14 positive patients in 466 patients without AP,and the specificity was 97%,positive predictive value was 70%,while the negative predictive value was 99%.The sensitivity and specificity of urinary trypsinogen-2 assay ELASA was 93% and 95%.The test strip and ELASA methods had excellent relevance in detecting urinary trypsinogen-2 (κ=0.92).The sensitivity of serum and urine amylase in diagnosis of AP was 65% and 49%,and the specificity was 95%.Conclusions Urinary trypsinogen-2 assay test strip was rapid,convenient and sensitive for the diagnosis of AP with low cost,which was worth applying in emergency department for patients with acute abdomen.
