Abstract

The authers analized the patency of long bypas graftings (length over 40cm) using with PTFE grafts and observed an intimal lining of PTFE by scanning electromicroscopy (SEM). 1) In 2 of 9 cases (22.2%), grafts were occlueded during 33.9 months follow-up. Etiology of both occlusive cases was aortitis and graft size was 6mm in diameter and 50cm in length. 2) In 7 ASO cases, one graft was thrombosed because of stopping of anticoagulant therapy, but salvaged by thrombectomy. Other grafts in 6 cases were patent in long term period. 3) SEM revealed that endothelial lining on the surface of PTFE graft which was used in axillo-femoral bypass with length over 60cm was limited in the anastomotic area and intimal hyperplasia was observed in the anastomotic area. Central portion of the graft was not coverd with endothelial cell and fibrin. These finding have suggested that anticoagulant therapy are neccessary forever in the cases of PTFE graftings.
