Abstract
According to the AO cervical classification [1], a B-type fracture involves the posterior ligamentous complex with pure bony (B1), disco-ligamentous (B2) or distraction-extension (B3) fractures. In these cases, a complete reduction of eventual displacements is mandatory, together with a combined anterior-posterior fusion to restore the anterior column and recreate the posterior tension band. If an anterior cervical decompression and fusion (ACDF) with a cage and eventual plating is the choice for a single-level pathology, different techniques can be used for the posterior instrumentation and fusion [2–5]. In this specific case, the author performed a C5–6 ACDF with a tantalum cage without plating, and a posterior instrumentation of the lateral masses of C5 and C6.
