Abstract

Most people affected by HIV and tuberculosis live in sub-Saharan Africa where young adults have the highest incidence of both diseases. HIV transmission routes are well established in most adults and young children but are sometimes unclear in older children and teenagers. Sexual abuse may play an important role in this age group. A 12-year-old boy presented to a hospital in rural northwest Cameroon with a one-year history of productive cough and weight loss despite multiple antibiotics. On consultation there were night sweats and decreased appetite no rigors or haemoptysis respiratory rate was 28 breaths per minute blood pressure was 80/60mmHg temperature was 36.6°C and pulse was 100 beats per minute. He appeared chronically ill with submandibular lymphadenopathy bilateral coarse breath sounds and mild clubbing. (excerpt)
