Abstract

Background: Heart failure is characterized by high rates of morbidity and mortality, particularly in the elderly. This illness profile underscores the need to consider the integration of palliative care principles in care planning. In spite of this, determining prognosis and the introduction of a palliative approach can be challenging. Aim: Deaths occurring in patients enrolled in a home-based heart failure program were reviewed to identify biochemical and demographic characteristics to assist in developing prognostic models.
