• Few pre-operative assessment guidelines are specific to obesity.• The purposes of pre-operative assessment are to improve the quality of care and maximise efficient use of theatre time.• Anaesthetists carry an overall responsibility for pre-operative assessment.• In preassessment protocols, morbidly obese patients should be allocated to a higher ASA grade than history alone suggests.• An anaesthetist experienced in treating morbidly obese patients should be involved in pre-operative assessment.
