Abstract

This chapter discusses some of the practical applications of the philosophies and methods of risk and reliability management, and some of the applications of the calculation methods. Most of the case studies are based on real cases in a number of countries, but they have been changed sufficiently to avoid identification of the particular facilities, and have been simplified to clarify the points being illustrated. The worked examples are also simplified to illustrate the methods more clearly. The features of the facility and its operation relevant to this case study are operation, supervision, maintenance, electrical and instrument maintenance, auditing, and emergency services. The important case studies discussed in the chapter are that of an LPG facility risk, a siting of a hydrocarbon processing plant, a risk analysis of a proposed LPG storage and processing plant, a siting of an LPG tanker loading a bay control room, resisting of a toxic gas plant, a review of an oil refinery, a review of a gas/liquid separation plant, faulty approach to a risk-based inspection, and legal obstacles to an auditor. The important worked examples mentioned are heat radiation risk, vapor cloud risk, toxic gas risk, separation distance estimation, separation distance estimation, fault tree analysis, and cutset analysis.
