Abstract

Abstract systemic PDT using first generation photosensitizers, e.g. porfimer sodium (Photofrin®), which induce prolonged generalized cutaneous photosensitivity, may provide still an option for multiple or large tumors, in particular basal cell carcinomas, in elderly patients unsuitable for surgery or any other therapeutic modality. From the literature a concentration of 2 mg/kg b.w. and irradiation with a fluence of approx. 100 J/cm2 (100–150 mW/cm2) using red light should be used for successful treatment. However, new photosensitizers absorbing in the near infrared part of the visible spectrum and not inducing prolonged cutaneous photosensitization are under investigation. Indications for systemic PDT with these drugs, whose target is primarily the microvasculature, will be probably unpigmented solid tumors with increased microvascular density and a tumor thickness
