Abstract

Abstract Date Presented 04/05/19 African American women diagnosed with breast cancer where this study was completed have significantly worse outcomes than Caucasian women. The purpose of this study was to prevent and manage chronic disease in AABCS. Partnering with the community to deliver interdisciplinary group intervention had a significant impact on changing health behaviors and risk factors to prevent and manage chronic conditions and reduce health disparities among AABCS.
