Abstract
Maddie, a 17-year-old patient with congenital melanocytic naevi who is a Young Ambassador with Caring Matters Now, wrote this article while on work experience with the Support Pathway Practitioner for the charity. This article discusses how young adult members should be supported while transitioning to adult services and the challenges faced by young people affected by a rare skin disease as they transition from specialized care to local services.
