Abstract

history of diabetes mellitus, and elevated maternal hemoglobin A1C value suggest that the mother may have had gestational diabetes despite the negative antenatal screen. Infants of diabetic mothers are known to be at increased risk for polycythemia2 ~ This patient's course emphasizes two related issues. (1) Newborn infants with hyperviscosity should have careful assessment of renal function, and if this is found to be significantly impaired, partial exchange transfusion may be indicated. (2) Babies with oliguria or renal failure without obvious reason should have a venous hematocrit measured to rule out hyperviscosity as an occult cause, even in the absence of more common symptoms.
